The Office of Institutional Research has provided this form for handling and tracking data requests by internal and external organizations. We appreciate your use of this form for all requests.

Our office strives to provide you with the information that you request in a timely manner. For this reason, it is very important that requests are made as far in advance as possible. Please allow at least three weeks for our office to review and complete your request.

Please describe in detail the report that is needed for this request. Also provide the semesters or time frame you need. The more detail that you provide, the quicker your request will be processed. If this is a recurring request, please provide the request number from the prior year.
